We engineered a centralized, GDPR-compliant platform for a leading Dutch online psychology clinic. By unifying fragmented tools, integrating state-mandated EHRs, and automating medical reporting, LMcare eliminated high licensing costs and drastically reduced administrative overhead.

To resolve these architectural and operational challenges, Apzumi engineered a robust, centralized middleware platform that serves as a single source of truth. Rather than building everything from scratch, the development team integrated existing critical systems and replaced redundant paid tools, creating a unified and secure ecosystem that's easy to use for both clients and medical staff.
Apzumi started by replacing the costly third-party video software with a comprehensive, cost-effective tool that synchronizes with doctors' calendars. Then, to streamline clinic management, we integrated the platform directly with LMcare's internal HR system. When a therapist submits a leave request, the platform automatically marks their absence in the calendar. The clinic is legally required to use a certified EHR system. Apzumi built a custom API integration that acts as a modern overlay on top of it. Our system automatically fetches client profiles and medical referrals directly from the EHR, eliminating manual data entry. Our overlay successfully centralized client management, drastically minimizing the time staff spend navigating the outdated state system. Ultimately, by eliminating cross-system data entry and centralizing workflows, the total time psychologists spend on administrative tasks has been more than halved.
Given the strict regulatory environment, security and compliance were embedded into the architecture from day one. Apzumi conducted rigorous GDPR risk analyses before implementing any third-party tools. We also architected encrypted AWS databases with strict IAM (Identity and Access Management) protocols, allowing the clinic to securely process and store medical reports and therapy notes while being fully compliant with data protection laws.
To further reduce the administrative burden, we successfully integrated a highly specialized, EU-based medical transcription tool. Immediately after a session, the system processes the audio and provides the psychologist with a formatted, compliant medical report draft ready for review. This frees up hours of work that psychologists would otherwise spend typing reports manually.
Finally, Apzumi introduced advanced client-facing tools, including daily mood tracking, journaling, and NuraLogix FaceScan integration. This data feeds into a custom Risk Monitoring engine that automatically alerts psychologists if a client's well-being score drops significantly, enabling proactive and data-driven mental healthcare. For psychologists this is a powerful supporting treatment tool, as they gain an unprecedented insight into clients' well-being on days without sessions. For clients, the intuitive UI/UX offers an engaging way to monitor their own progress, resulting in an outstanding client satisfaction score of 8.9. Additionally, thanks to the system of smart reminders, the client attendance rates were driven to over 97%.
What proves the platform's thoughtful architecture and intuitive design is the fact that LMcare was able to seamlessly migrate 80 psychologists and 400 active clients to the new system without a single operational hiccup.
Before partnering with Apzumi, LMcare's operations were severely bottlenecked by a fragmented IT infrastructure. Psychologists had to navigate multiple applications: an isolated national Electronic Health Record (EHR) system, standalone HR software, ad-hoc client communication channels, and premium third-party video conferencing tool. None of these applications communicated with each other.
This lack of interoperability created massive operational overhead. Aside from focusing fully on patient, psychologists were spending an extensive amount of time on administrative tasks, copying data entry between systems, and documentation. Additionally, the clinic was paying significant licensing fees for a video tool that wasn't seamlessly integrated into their core workflows.
LMcare utilizes their own proprietary, intensive therapy model (the FIT™ method), which enforces strict scheduling rules and custom triage processes. As the model is unique, off-the-shelf software could not accommodate its business logic, leading to scheduling conflicts. Lastly, from a compliance perspective, operating in the Netherlands - a member state of the European Union - meant that any new system architecture had to strictly adhere to GDPR regulations, ensuring the highest standards of data encryption and secure handling of psychological records.

In this phase we mapped the fragmented existing stack, captured legal and GDPR requirements, and documented core therapy workflows to define an MVP backlog. We carried a discovery workshop with C-level executives, therapists and administrators, and created a list of critical requirements and needed integrations.
We designed the flow for 3 types of core users - psychologists, patients, and admins. The results were flows for psychologist web portal (with dashboards, sessions notes, and caseload), for patients for both web and mobile (onboarding, booking, journaling) and for admin portal (user management, audit views).
In this stage, we focused on delivering the client-facing mobile application in a hybrid approach for both iOS and Android. Aside from adding features like authentication, push notifications, mood tracking and journaling, we created a working integration with a video tool and with NuraLogix FaceScan.
In this stage, we focused on delivering a full-stack web platform with tailored interfaces for psychologists, patients, and admins. Our focus was on ensuring GDPR compliance with e.g. rolebased access or secured clinician-patient messaging. The platform handles the entire clinical lifecycle, from automated session scheduling and video conferencing to secure documentation and EHR synchronization.
We conducted a full testing cycle, completed a GDPR compliance audit, and launched a pilot group to validate the platform in a real-world environment. Alongside bug fixing, data migration verification, and performance monitoring, we gathered user feedback, finalized the release plan, and prepared operational documentation and staff training for the full rollout.
After the initial launch, we focused on stabilizing the platform and monitoring daily operations. We are now continuously improving the product by adding new features and updating the roadmap based on actual user needs and performance data.
LMcare TrueTrail is a comprehensive healthcare platform consisting of Angular 16.2 web portals, a Flutter-based mobile application, and a Java 21/Spring Boot 3.4 REST API backend. The system is deployed on AWS infrastructure managed via Terraform, utilizing RDS PostgreSQL for data storage, AWS Cognito for authentication, and AWS SQS for asynchronous processing. It integrates with external services such as Firebase Cloud Messaging, and an EHR system to provide a streamlined management of medical data and patient care.
